LAMINOPLASTY

Laminoplasty is spinal decompression surgery that is done to relieve pressure around the spinal canal, surrounding structures and nerves.

Laminoplasty

First, the lamina is cut through, like opening the door to make more room and alleviate pressure. At the same time, surrounding debris is removed to reduce pressure further, and a hinge in the lamina is created. Finally, the other end of the hinge is stabilised with a metallic plate or bone strut. Surgery of this nature is common in the cervical spine.

When is a laminoplasty required?

A laminoplasty is required should symptoms of spinal stenosis occur, which includes pain, a tingling sensation and numbness in the area. This surgery is also requested when myelopathy develops. Myelopathy occurs due to damage to the spinal cord, causing compression to the cord and nearby structures—myelopathy results in issues related to balance, dexterity and bowel dysfunction.

Procedure

Dr Makan creates a cut along the back of the neck and uses retractors to pull fat and muscle away, so he is careful not to remove the bone and other compressive tissue but instead uses a drill to thin the bone that spreads over the spinal cord. Afterwards, Dr Makan shifts the lamina to one side and secures it using a hinge. As a result, there is more room for the spinal cord. Next, everything around the spinal cord obstructing it, including herniated discs and bone spurs, is removed. Lastly, Dr Makan secures the newly made door of the lamina with a metal plate that remains open.

Recovery

A drain may remain at the incision site to remove excess blood or fluid. At a follow-up consultation, Dr Makan will remove the drain. Any sutures or staples that are non-dissolvable will be removed too. Usually, after surgery, a hospital stay over two days is requested. Full mobility is mainly dependent on age, health status and outcome of the procedure.

FAQ

1What can you expect after a laminoplasty?

You are encouraged to walk gradually but not to overexert yourself. Avoid taking pain medicine while driving and do not carry heavy items. Remain active and try not to lie down too much as this will worsen pain and stiffness.

2Can you perform a laminoplasty less invasively?

There are less invasive surgical techniques used when performing a laminoplasty. This less invasive means of surgery involves fewer, smaller incisions and a faster recovery.

3How effective is a laminoplasty in treating cervical spondylotic myelopathy?

An open door expansile laminoplasty is a safe and effective alternative to surgery from an anterior approach. A laminoplasty treats cervical spondylotic myelopathy effectively with very little blood loss and carries minor risks.
